AOD-9604 is an experimental peptide derived from a specific portion of human growth hormone (HGH). It was developed to investigate certain metabolic effects of HGH without producing the same growth-related hormonal activity.

Although much of the discussion around AOD-9604 focuses on fat metabolism and body composition, understanding its safety is equally important.

Reported AOD-9604 side effects in clinical studies included headaches, nausea, diarrhea, dizziness, fatigue, and other gastrointestinal symptoms. However, many of these events also occurred in placebo groups, making it difficult to determine whether AOD-9604 caused them.

A 2013 review of six clinical trials involving 893 participants reported generally favorable tolerability. Nevertheless, the FDA has raised additional concerns about compounded AOD-9604, including potential immune reactions, peptide impurities, and limited safety information for certain administration routes.

This article explains what researchers have observed, which side effects remain uncertain, and why AOD-9604 has not received FDA approval.

What Is AOD-9604?

AOD-9604, or Anti-Obesity Drug 9604, is a modified fragment of human growth hormone.

It is based on amino acids 177–191 of HGH, with an additional tyrosine residue.

Researchers developed AOD-9604 to examine whether the lipid-metabolism effects of HGH could be separated from its growth-promoting properties.

Early experimental investigations focused on processes such as:

  • Lipolysis, the breakdown of stored triglycerides.
  • Fat oxidation and energy metabolism.
  • Lipogenesis, the formation and storage of fat.
  • Metabolic pathways related to growth hormone activity.

However, its clinical development as an obesity treatment was discontinued after a larger study failed to demonstrate a significant weight-loss benefit.

AOD-9604 is not an FDA-approved medication, and there is no established therapeutic dosing regimen.

What Are the Most Commonly Reported AOD-9604 Side Effects?

Clinical trials provide the most useful information about adverse events observed after AOD-9604 administration.

The 2013 safety review by Stier and colleagues evaluated six randomized, double-blind, placebo-controlled trials conducted between 2001 and 2006.

Across these studies, researchers recorded several types of adverse events.

Reported adverse eventFindings from clinical research
HeadacheFrequently reported in several studies, including placebo groups
NauseaReported during oral administration studies
DiarrheaObserved in multiple trials, with one serious episode considered possibly treatment-related
DizzinessReported during an early intravenous study
FatigueObserved in early clinical investigations
FlatulenceReported in oral administration studies
Injection-site discomfortReported during intravenous administration studies
Chest tightnessOne severe event was considered possibly treatment-related in an early intravenous study

An important distinction is that an adverse event occurring during a trial is not automatically a side effect caused by the substance being studied.

Researchers examine the timing, severity, frequency, and distribution of events between treatment and placebo groups to assess possible relationships.

1. Headaches

Headache was one of the most frequently reported adverse events during AOD-9604 clinical research.

In an early intravenous study involving 23 participants with obesity, 16 participants reported headaches of mild or moderate intensity.

Headaches were also common in larger oral administration studies.

For example, the 24-week trial recorded headache events in approximately 25.9% of participants during the active treatment phase.

However, the distribution of adverse events was broadly similar across AOD-9604 and placebo groups.

What this means: Headaches were common during the studies, but the evidence does not establish that AOD-9604 consistently causes headaches more frequently than placebo.

2. Nausea and Digestive Discomfort

Gastrointestinal symptoms were another category of events reported in clinical studies.

These included nausea, diarrhea, flatulence, abdominal discomfort, and changes in appetite.

A study examining single oral doses of AOD-9604 recorded several digestive symptoms.

In the seven-day study, participants receiving the highest investigated oral dose experienced more headaches, diarrhea, and flatulence than participants in some lower-dose groups.

However, the researchers did not observe a consistent adverse-event pattern across all treatment groups.

These findings indicate that digestive symptoms occurred during exposure, but their exact relationship to AOD-9604 remains uncertain.

3. Diarrhea

Diarrhea deserves separate attention because it was observed in several studies and was associated with one potentially treatment-related serious adverse event.

During an early oral dose-escalation trial, a participant experienced a serious episode of diarrhea after receiving a 54 mg dose.

The investigators classified the event as possibly related to treatment.

This does not prove that AOD-9604 caused the episode, but it is a relevant safety finding.

In the larger 24-week clinical trial, diarrhea was reported in approximately 7.8% of participants during the treatment phase.

As with headaches, the overall event pattern did not show a clear relationship to AOD-9604 compared with placebo.

4. Dizziness and Fatigue

Dizziness and fatigue were reported during the early intravenous AOD-9604 investigations.

In the first study involving healthy male volunteers, researchers documented:

  • Four fatigue events.
  • Three dizziness events.
  • One lethargy event.

Most adverse events were mild, and their distribution did not establish a consistent treatment-related pattern.

These findings cannot be used to estimate how frequently dizziness or fatigue might occur with repeated subcutaneous injections, since that administration method was not adequately investigated in the major published trials.

5. Injection-Site Reactions

Injection-site symptoms were recorded in early studies involving intravenous AOD-9604 administration.

Reported events included injection-site pain, bruising, and an application-site reaction.

However, these studies did not establish the adverse-event frequency for repeated subcutaneous administration.

That distinction matters because intravenous infusions and subcutaneous injections differ in how a substance enters the body and how surrounding tissues are exposed.

Potential complications associated with injections generally include local irritation, infection, and tissue injury, especially when products are improperly manufactured or administered.

These are general injection-related risks rather than confirmed AOD-9604-specific adverse effects.

Can AOD-9604 Cause Serious Side Effects?

The historical trials did not identify a consistent pattern of serious adverse events attributable to AOD-9604.

However, serious medical events did occur during the studies.

In an early oral study, one serious diarrhea event was assessed as possibly treatment-related.

An early intravenous study also recorded a severe episode of chest tightness considered possibly related to treatment.

Later, larger trials included serious adverse events such as cancers, injuries, infections, and other medical conditions.

The investigators generally did not consider these events causally related to AOD-9604.

It is important not to confuse these findings.

An event being reported during a clinical trial does not mean the experimental compound caused it. Equally, investigators finding insufficient evidence of causation does not establish that the compound is harmless.

The FDA has separately identified serious adverse-event reports that may be associated with AOD-9604, while noting that causality remains unclear.

Does AOD-9604 Affect Blood Sugar or Insulin Sensitivity?

Human growth hormone can influence insulin sensitivity and carbohydrate metabolism.

Because AOD-9604 is derived from HGH, researchers investigated whether it produced similar effects.

The 2013 safety review examined glucose-related measurements, including:

  • Fasting blood glucose.
  • Serum insulin.
  • Oral glucose tolerance testing.
  • Changes in metabolic parameters during treatment.

The reviewed studies generally did not demonstrate clinically meaningful adverse effects on glucose tolerance compared with placebo.

However, these findings should not be interpreted as evidence that AOD-9604 improves glucose regulation.

There is insufficient clinical evidence supporting AOD-9604 as a treatment for diabetes or insulin resistance.

Furthermore, its glucose-related safety in people with significant metabolic disease or during long-term injectable administration remains uncertain.

Can AOD-9604 Increase IGF-1 Levels?

Insulin-like growth factor 1 (IGF-1) plays an important role in growth and cellular development.

Full-length HGH stimulates IGF-1 production, which contributes to many of its biological effects.

Researchers examined IGF-1 levels during AOD-9604 trials to determine whether the fragment retained this growth-related activity.

Across the reviewed studies, AOD-9604 did not significantly increase serum IGF-1 levels compared with placebo.

This supports the understanding that AOD-9604 has a different biological activity profile from full-length human growth hormone.

Nevertheless, the absence of significant IGF-1 elevation does not establish that AOD-9604 is safe for every population or administration route.

Can AOD-9604 Trigger an Allergic or Immune Reaction?

Immunogenicity refers to the ability of a substance to stimulate an immune response.

Peptides may provoke immune reactions depending on their molecular characteristics, purity, formulation, and route of administration.

During historical clinical research, investigators examined blood samples for antibodies directed against AOD-9604.

No anti-AOD-9604 antibodies were detected in the selected participants tested.

Although this was a favorable observation, the studies had limitations.

Most longer-term investigations used oral formulations rather than repeated subcutaneous injections.

Injectable peptides may present different immunogenicity concerns because the substance and any associated impurities are introduced through a different route.

The FDA has specifically identified potential immunogenicity risks associated with compounded AOD-9604 for certain administration routes.

This means the absence of detectable antibodies in historical trials cannot eliminate the possibility of immune reactions with other formulations.

Symptoms such as difficulty breathing, throat swelling, or rapidly developing facial swelling after exposure to any substance require immediate emergency medical attention.

Are AOD-9604 Injection Side Effects Different From Oral Side Effects?

The route of administration can influence how a substance is absorbed, distributed, and tolerated.

AOD-9604 clinical research primarily examined oral administration and single intravenous infusions.

Subcutaneous injections are frequently discussed online, but the published human evidence supporting their safety is limited.

Oral Administration

The largest clinical studies investigated oral formulations over periods of up to 24 weeks.

These studies provided information about gastrointestinal symptoms, headaches, glucose regulation, IGF-1 levels, and laboratory safety measurements.

However, oral administration did not demonstrate consistent therapeutic effectiveness.

Intravenous Administration

Early clinical trials investigated single intravenous infusions under controlled research conditions.

Reported events included headaches, fatigue, dizziness, and local administration-site symptoms.

These findings offer limited information about the potential consequences of repeated exposure.

Subcutaneous Administration

There is insufficient high-quality human evidence to determine the frequency or severity of adverse effects from repeated subcutaneous AOD-9604 injections.

Potential concerns include immune reactions, local tissue responses, contamination, and formulation-related impurities.

Safety results obtained from oral or intravenous studies cannot automatically be applied to subcutaneous administration.

What Are the Long-Term Side Effects of AOD-9604?

Long-term safety remains one of the most important unanswered questions.

The major clinical studies were conducted more than two decades ago, with the longest controlled treatment periods lasting approximately 24 weeks.

Although researchers observed generally favorable tolerability in those trials, they did not establish the effects of prolonged use over multiple years.

Important unresolved questions include:

  • Whether prolonged exposure affects immune responses.
  • Whether repeated injections increase the risk of sensitization.
  • Whether peptide impurities contribute to adverse effects.
  • Whether long-term administration influences endocrine function.
  • Whether specific populations are more vulnerable to complications.

There is insufficient evidence to make reliable claims about the long-term safety of AOD-9604.

Does AOD-9604 Have the Same Side Effects as Human Growth Hormone?

AOD-9604 and full-length HGH are not the same substance.

Full-length HGH has established biological actions involving growth, metabolism, and IGF-1 signaling.

Its recognized adverse effects can include fluid retention, joint discomfort, and changes in glucose regulation, depending on the clinical circumstances.

AOD-9604 was designed to investigate certain metabolic effects without reproducing the entire activity of HGH.

Historical clinical studies did not demonstrate significant IGF-1 elevation or consistent impairment of glucose tolerance.

However, this does not mean AOD-9604 is free from hormonal or metabolic risks.

Its overall clinical safety has not been established to the standards required for an FDA-approved medication.

Is AOD-9604 Safe for Compounding?

The FDA has raised concerns about compounded medications containing AOD-9604.

Compounding involves preparing a medication for a particular patient or clinical requirement. However, compounded drugs are not FDA-approved, and the agency does not review each compounded preparation for safety, effectiveness, or quality before marketing.

The FDA has identified several potential problems associated with AOD-9604-related substances.

Peptide Impurities

Peptide manufacturing can produce unwanted molecular fragments or related substances.

These impurities may create additional safety risks, especially when the final product has not undergone appropriate characterization and quality testing.

Aggregation

Peptide molecules may aggregate or form larger molecular structures under certain conditions.

Aggregation can affect stability and potentially influence immune responses.

Incomplete Safety Information

The FDA has identified limited safety information for some proposed administration routes.

Without adequate human clinical data, the risks of those formulations cannot be reliably determined.

Uncertain Product Quality

Products marketed online may differ in purity, identity, formulation, and manufacturing controls.

A label identifying a substance as AOD-9604 does not independently establish that its contents meet pharmaceutical quality standards.

For these reasons, patients should not assume that compounded or commercially marketed AOD-9604 products have the same characteristics as the investigational formulations used in historical clinical trials.

Is AOD-9604 FDA-Approved?

No. AOD-9604 is not FDA-approved for weight management, metabolic health, hormone replacement, or any other therapeutic indication.

It has no FDA-approved prescribing information or established dosage schedule.

The FDA reviewed AOD-9604-related bulk drug substances in connection with its pharmacy compounding evaluation process, including discussion at the December 4, 2024, Pharmacy Compounding Advisory Committee meeting.

The agency’s safety concerns include the potential for immune reactions, peptide-related impurities, limited safety data, and serious adverse-event reports with uncertain causality.

Its investigational history does not mean it has met FDA standards for routine medical use.

Frequently Asked Questions About AOD-9604 Side Effects

What is the most common reported AOD-9604 side effect?

Headache was among the most frequently reported adverse events in human clinical studies. However, headaches also occurred commonly in placebo groups, so a direct causal relationship has not been consistently established.

Can AOD-9604 cause nausea?

Nausea was reported during oral AOD-9604 studies, along with other gastrointestinal symptoms. The evidence does not establish a consistent increase in nausea compared with placebo.

Does AOD-9604 cause diarrhea?

Diarrhea occurred in several trials. One serious diarrhea episode was considered possibly related to AOD-9604 treatment. Most gastrointestinal events were mild or moderate, but the available evidence does not establish the precise risk for every formulation.

Can AOD-9604 cause cancer?

Certain cancers were reported as serious adverse events during historical clinical trials, but investigators did not consider them causally related to AOD-9604. The available evidence does not establish that AOD-9604 causes cancer, and it is insufficient to rule out every potential long-term risk.

Does AOD-9604 affect hormones?

In the reviewed human trials, AOD-9604 did not significantly increase IGF-1 concentrations. However, its complete endocrine effects, particularly during prolonged or repeated injectable administration, are not fully established.

Can AOD-9604 cause an allergic reaction?

Potential immune reactions are a concern with peptide formulations. Historical trials did not detect anti-AOD-9604 antibodies in selected participants, but the FDA has identified immunogenicity concerns for certain compounded formulations and administration routes.

Is AOD-9604 safer than HGH?

AOD-9604 produced a different response from HGH in certain clinical measurements, including IGF-1 and glucose tolerance. However, there is insufficient evidence to conclude that it is categorically safer than FDA-approved HGH therapy.

Is AOD-9604 safe for daily use?

Daily oral administration was investigated in controlled trials lasting up to 24 weeks. These findings do not establish that daily AOD-9604 administration is safe or appropriate for general clinical use, particularly through subcutaneous injection.

Final Thoughts: What Should Patients Know About AOD-9604 Side Effects?

AOD-9604 has undergone human clinical testing, and the available studies provide useful information about its tolerability.

Headaches, gastrointestinal symptoms, dizziness, fatigue, and other adverse events were reported during clinical investigations.

However, many events also occurred in placebo groups, and most studies did not demonstrate a consistent treatment-related adverse-event pattern.

The historical findings should be considered alongside important limitations.

The FDA has identified potential safety concerns involving compounded AOD-9604, including immunogenicity, peptide impurities, and inadequate information about certain administration routes.

There is also no FDA-approved AOD-9604 treatment protocol, and its long-term safety remains uncertain.

At Optimal HRT, we believe decisions about hormone and metabolic health should be based on established clinical evidence and individualized medical evaluation. Understanding both the potential risks and the limitations of available research helps patients make informed decisions about their care.
